Job Description

The Nurse Coordinator serves as a member of a multi-disciplinary care team and collaborates with physicians, surgeons, and APPs to screen, assess, evaluate and determine the appropriate plan of care for patients. Coordinates overall care of the patient population by incorporating current standards of care in the outpatient setting. Serves as the liaison between the providers and patient/family and has direct patient contact. Develops and maintains protocols, policies, and procedures in collaboration with medical providers and hospital management.

  • Works closely with APPs, surgeons and physicians in assessment, evaluation and treatment of patients

  • Triages incoming referrals, and reviews records to determine needed diagnostic testing according to established protocols

  • Serves as an interdepartmental liaison for procedure scheduling and a point of contact for physicians and APPs

  • Coordinates optimal patient and family experiences from diagnosis, evaluation, treatment and follow-up

  • Educates patients regarding treatment plans, schedules procedures and provides follow-up

  • Maintains detailed records regarding referrals and treatment plans

  • Prepares PowerPoint presentations and assists with patient case review at weekly hospital conferences

  • Collaborates with vendors and transmits patient data and images for case reviews

  • Develops and maintains protocols, policies and processes in collaboration with clinic management and medical providers, and in accordance with SLH policies

  • Analyzes patient flow problems and assists in instituting corrective actions

  • Integrates health information technology to improve quality and cost-effectiveness of patient care

  • Promotes delivery of timely, high-quality, high-value care across the continuum of care

  • Attends educational conferences to enhance knowledge of patient population as well as current and investigational treatments

Job Requirements

Applicable Experience:

2 years

Basic Life Support – American Heart Association or Red Cross, Registered Nurse (MO) – National Council of State Boards of Nursing (NURSYS)

Bachelor’s Degree
