Job summary:

The Case Manager will work as part of a multi-disciplinary team to facilitate patient care by assessing patient needs, evaluating service options, coordinating care, and gauging client progress.


Qualifications:

Required:

  • Associates degree in human services or a minimum of two (2) years’ experience in a related field
  • Actively working toward a State of Alaska Peer Support Professional certification. To be acquired within 6 months of date of hire
  • Lived experience in addiction and recovery (personal or familial)
  • Understanding of trauma informed care. Including the use respectful, person-centered, recovery-oriented language in written and verbal interactions
  • Ability to pass a drug screening and State of Alaska background check
  • A current valid driver’s license
  • Qualify for SFA’s commercial automobile insurance

Skills required:

  • Great written and verbal communication skills
  • Punctuality reporting to work and in delivery of direct services
  • Ability to:
    • Establish and maintain clear therapeutic boundaries
    • Work as a team member including accepting and providing supervision
    • Build rapport with clients and staff
    • Tell their recovery story and use their lived experiences to inspire and support clients living with behavioral health conditions
  • Basic computer skills including Internet navigation and fluid use of electronic communication tools

Job responsibilities:

  • Assist potential clients in the completion of the intake packet, connect clients to resources necessary to complete the intake process, notify clients of acceptance, and coordinate a day and time for admission
  • Initiate and develop ongoing relationships with clients
  • Work with clients to:
    • Develop an Individual Action Plan (IAP)
    • Assess needs and potential barriers to success
  • Provide:
    • Support services, crisis intervention, medication monitoring and education as needed
    • Employment development and job readiness education to assist client in obtaining work and/or schooling
    • Life-skills based groups to encourage the balance of sobriety and functioning as a healthy community member
    • Advocacy for clients; educating them, their families, and significant others regarding mental health and/or substance abuse issues and the impact on their lives
  • Establish and maintain an active list of resources available in the community, and foster working relationships with providers of those services
  • Coordinate with service providers, manage client cases, and facilitate access to services in the community which promote the physical, mental, and spiritual health of the client
  • Participate in team decisions, advising the team of pertinent information
  • Communicate and document client progress toward recovery goals, including all services provided as part of the IAP
  • Collaborate with the Peer Support team to coordinate care for clients, their families, healthcare providers, etc.
  • Assist in the discharge and transition of clients into the community
  • Maintain confidentiality of all information as it relates to the position

Supervision:

  • Recovery Residence Case Manager will receive direct supervision from the Recovery Residence Manager
